Please note that by accepting Terms & Conditions and using the software (the “Eve”) and/or the services provided by Bloomfield Technology (“Company”, “we”, “our” or “us”) including (but not limited to) Bloomfield Technology web platform, and/or Bloomfield Technology mobile app, you (a “user”, “member” or “participant”) are consenting to the collection, use, and disclosure of your Personal Information (defined below) in accordance with this Privacy Policy. If you do not agree to this Privacy Policy, please do not use the software or Company’s services.

Your consent will be effective from the date you confirm acceptance (i.e., the date when you sign the Pilot program Agreement and or Employee Consent Form) of our Terms & Conditions and use the Eve until the end of the Pilot Program Period. If you provide consent to the collection, use and disclosure of your Personal Information as described in this Terms & Conditions and Privacy Policy, Bloomfield Technology will be able to provide you with access to a personalized experience to obtain a health and wellness information and related results, tips and education from our health and wellness partners. You may withdraw your consent at any time. However, if you choose to withdraw your consent, we will not be able to provide you with the functions or services through our application.

This information is being provided to you so that you can make an informed decision about seeking the services of Bloomfield Technology. Please ensure you carefully read this information. By Engaging this service, you acknowledge and agree to the Disclaimer, Limitations of Liability and associated risks outlined below.


The services provided by Bloomfield Technology are different from the standard consultation typically sought locally through a medical service provider, as it does not directly obtain your medical history or conduct a physical examination. In view of these limitations, the health and wellness data, and related scores and notifications provided should be taken as the best judgment based upon information available.


You will be required to upload your information and biometric data either manually or in an automated fashion. The Bloomfield Technology software will ingest the provided data/information and present the results in the form of emails, management dashboard and personalized scores that are visible on the tablet.


By engaging services through Bloomfield Technology you agree to be bound by the following limitation of liability:

Bloomfield Technology commits to providing services in accordance with the Terms & Conditions on an as-is, where-is basis without warranties of any kind to the maximum extent permitted by applicable law. You acknowledge that these services do not create a doctor-patient relationship, are not a substitute for regular medical care, and are not guaranteed to be error-free. You acknowledge that the information provided to you is intended to be shared with your Employment company, depending on the Anonymity settings you use. Bloomfield Technology and its technicians, partners, employees, and contractors do not represent or warrant the suitability of the information provided for any general or specific use and assume no liability or responsibility of any kind in connection with such information.

In no event shall Bloomfield Technology or its Directors, technicians, partners, employees and contractors be liable to you or to any third party for any consequential, incidental, indirect, exemplary, special or punitive damages whether arising out of breach of contract, tort (including negligence, malpractice and improper treatment of personal information) or otherwise, regardless of whether such damage was foreseeable and whether or not such party has been advised of the possibility of such damages, and notwithstanding the failure of any agreed or other remedy of its essential purpose.

By requesting health and wellness monitoring through Bloomfield Technology, you acknowledge and agree that:

  • You are at least the age of majority in the State or Territory of your residence and are entitled to enter into this agreement with Bloomfield Technology;
  • Bloomfield Technology services are not being sought for a medical emergency or a condition requiring urgent medical attention;
  • The consultation provided is not intended to represent or replace a standard medical assessment sought for diagnosis, management and follow up of a new or known medical condition;
  • Engaging Bloomfield Technology does not constitute establishing a binding patient-doctor relationship with duty to provide care;
  • You have read, understood and agree to be bound by the Terms & Conditions and Limitation of Liability, above, as well as the Terms & Conditions and Privacy Policy of Bloomfield Technology.
  • You agree to provide the necessary information as requested by Bloomfield Technology
  • You consent to Bloomfield Technology contacting you via email, phone and or other means of communication.
  • I consent to receive emails from Bloomfield Technology containing results, tips and education regarding Bloomfield Technology.

We recognize that when you provide us with information about yourself that you trust us to act in a responsible manner with that information. Personal Information is any information that identifies you, or by which your identity can be deduced. This Privacy Policy discloses the Company’s Personal Information gathering and dissemination practices with respect to the Site and our services. Please read this Privacy Policy carefully.

Information Collected and How It Is Used

The following describes the types of Personal Information and other information the Company may collect about you, and how the Company may use and maintain that information, including, but not limited to:

  • Registration

    Before you can utilize certain services that we offer, we may ask for some personal information including your email address, name, job department, contact and personal details. For the purpose of the Pilot, and to minimize the organizations tasks, we will register users based on the information provided to us either through a registration tool or through the organization. We request this information for identification purposes, to communicate with you regarding your account, and to facilitate the functioning of certain services. We may maintain certain Personal Information about you as long as reasonably necessary for us to complete our dealings with you, or as may be required by law.

  • Facial Data

    Eve uses AI software which determines the wellness measures stated in the product brief. This is done through a live stream and no face or facial data is stored, saved or kept. Numerical data from the live streams is delivered via API and displayed in the Front-End application. Eve does not use any face recognition processes nor does it save or store user’s facial data in any way.

  • Forms

    To fully utilize the services we offer, you may be required to fill out forms that will contain Personal Information such as name, address, date of birth, health and medical information, business department and/ other details relating to the information obtained from your company which are necessary information during the Pilot Program Test period. Customer may opt to put “Anonymous” as their name, but we will need the Customer’s email address so the system could send your tips and education. Once the results populate in the dashboard, your email address will not show up in the team dashboard and your details will be anonymous in the system as well.

  • Correspondence

    If you correspond with us via email, we may gather in a file specific to you the information that you submit in order to communicate with you effectively and respond to any of your inquiries. We maintain certain Personal Information about you as long as reasonably necessary for us to complete our dealings with you, or as may be required by law.

  • Location

    We collect information about your location in order to provide you certain functionalities of the Service. We will collect your location where it is necessary for the purposes of providing you with the location-based services features of the app as well as for the legitimate interests of Bloomfield Technology, such as for health data analytics, while the application is running. You can turn off location sharing – if you do so, we will continue to collect your location information in order to provide you the Service. You can also stop sharing your location with us at any time by updating the settings on your mobile device to limit the app’s access to your location information. Please note, however, that we may not be able to provide all of the functionalities of our Services to you if you disable access to location data. If you no longer want to continue using the Service, uninstalling the Bloomfield Technology app or logging out from the Service will also stop us from collecting your location.

  • Choices about your information

    Customer may opt to put “Anonymous” as their name, but we will need the Customer’s email address so the system could send your results, tips, and education. If you opt for “Anonymous”, your email address will be blocked from view in the Admin Dashboard.

Information Disclosure and Usage

Bloomfield Technology will not share, rent, sell or otherwise disclose any of the Personal Information that we collect about you, except when we have your permission or in any of the following situations:

We may disclose Personal Information that we collect about you to our third-party contractors who perform services for Bloomfield Technology in order to provide certain services or to complete or confirm a transaction that you conduct with us. We require these third-party contractors to use reasonable safeguards to protect your Personal Information in their custody or control. Occasionally we use service providers located outside of your local jurisdiction to process and/or store Personal Information on the Company’s behalf. Please note that the Personal Information in the custody of these service providers may be subject to access by the law enforcement authorities of those jurisdictions in which the service providers are located.

Please contact us if you wish to enquire about more information on the specific location of your data server at any given time, and the associated safeguards in place in that jurisdiction.

We may disclose information regarding the results of the Pilot Program in terms of accuracy, uptake, errors, conclusions and feedback given to us by our Pilot Partner. We may use the conclusions drawn from the Pilot as part of our commercial operations including marketing, sales and business development.

Bloomfield Technology does not guarantee that any entity receiving such information in connection with one of these transactions will comply with all terms of this policy. We may disclose your Personal Information for purposes such as to provide services available on the Eve; to coordinate with insurance, reinsurance and excess or stop loss insurers; to enforce our members’ rights; to protect against actual or potential fraud; to resolve member inquiries or disputes; to receive payments; to carry out our business; to protect the confidentiality or security of our records; to administer preventive health and case management programs; to perform underwriting, auditing and rate making functions; to enable our service providers to perform marketing services on our behalf and inform members about our own products or services.

Security measures employed by Bloomfield Technology

The Eve requires users to provide us with their email address in order to log into the software. We utilize the emails to communicate their personal results, tips and educations and updates. To help protect the privacy of data you transmit through this Site, where Personal Information is requested: 1) Data that is transmitted over the network from the application to the mongo cluster is encrypted. 2) Using the https protocol is that data that is transmitted over the internet using this protocol is encrypted by the https principles. 3) We can add specific IP addresses from which we can reach the cluster, we can also configure roles on the atlas in order for us to prohibit certain users from doing data manipulations, for example, deleting and editing. In addition, Company takes other reasonable physical, technical and organizational steps to protect the user data we collect against unauthorized access. However, you should keep in mind that the Eve and our services are run on software, hardware and networks, any component of which may, from time to time, require maintenance or experience problems or breaches of security beyond our control.

Please also be aware that despite our best intentions and the guidelines outlined in this Privacy Policy, no data transmission over the Internet or encryption method can be guaranteed to be 100% secure.

Bloomfield Technology takes no responsibility for any Cyber-attacks or security breaches into the Users and Clients organisation / business through our technology software, hosting and dashboard and any other technology platform.

Accessing, updating, or making corrections to information

You may access, correct or update Personal Information collected about you by managing your account profile or by contacting us at the email or mailing address noted below. We will use reasonable efforts to update our records. For our records, we may retain original and updated information for reasons such as technical constraints, dispute resolution, troubleshooting and agreement enforcement.

Your rights to access your Personal Information are not absolute, and the Company may deny access when: denial of access is required or authorized by law, when granting you access would have an unreasonable impact on other member’s privacy (unless we can sever the requested information from the Personal Information of others), and to protect our confidential commercial information. If we deny your request for access to, or refuse a request to correct information, we will explain why.

Changes to this Privacy Policy

Your continued use of the Eve and our services following an update or change means that you provide your consent to the collection, use and disclosure of your Personal Information as set out in the Privacy Policy. We reserve the right to update this Privacy Policy from time to time. Please visit this page periodically so that you will be apprised of any changes.

Age restriction

We do not knowingly collect or maintain Personal Information from persons who have not reached the age of majority in the Province or Territory of their resident (“Minors”), and no part of our system is directed to Minors. IF YOU ARE A MINOR, PLEASE DO NOT USE OR ACCESS OUR SITE AT ANY TIME OR IN ANY MANNER. If the Company learns that Personal Information of Minors has been collected without verifiable parental consent, then the Company will take the appropriate steps to delete this information. If you are a parent or guardian and discover that your child who is a Minor has obtained a Company account, then you may alert the Company at the address below and request that we delete that child’s Personal Information from our systems.

Additional Application

To be able to fully track the function and operation of the software, we will also install an application known as “anydesk”. Anydesk is a remote desktop application for us to be able to remotely check and follow the use of the system and device. Access to the device could be recorded for the sole purpose of checking and logging the issues of the system.

Rights and Restrictions

Customer is only authorized to use the Product at the install site for the length of the Pilot Program Period. If the Customer would like to use the Product for any purpose other than that outlined in the Pilot Agreement, they must inform Bloomfield Technology of their intended purpose.


Bloomfield Technology hereby grants the Client and users a non-sublicensable, non-transferable, non-exclusive license to perform and display the software Product, in object code form only, solely in accordance with documentation and materials supplied by Bloomfield Technology, and only for the duration of the Pilot Program Period.


User agrees not to:

  • rent, sell, lease, pledge, encumber, allow any lien or otherwise transfer the Product or any part thereof or use either for the benefit of any third party;
  • reverse assemble, reverse compile or reverse engineer the Product, or otherwise attempt to discover any Product source code or underlying Confidential Information;
  • merge the Product with any other software;
  • use the Product on or with any system for which it was not intended (pre-installed software Product shall remain in hardware Product);
  • use the Product in a way which is not expressly authorized hereunder or which may be illegal.
Ownership of the Products and Services

The Eve Product and the Services, together with all of the intellectual property and proprietary rights embedded within or arising from the Eve Product and the Services are the sole and exclusive property of Bloomfield Technology. Clients and Users/Customer acquires no ownership interest in the Eve Product or the Services from this Agreement or its use of the Eve Products and Services hereunder except for its limited use and access rights during the Pilot Program Period. All working from home participants with downloaded Eve applications must be immediately removed upon completion of the Pilot Program
